Participation and Medical Consent
As parent or guardian, I hereby give permission for my child to attend Adventure Clubs at Lakeside Bible Church. In the event of accident or injury, I will not hold Adventure Clubs, Lakeside Bible Church, or individual ministers or leaders responsible. I do herewith authorize treatment for the above minor in the event of a medical emergency which, in the opinion of the attending physician, may endanger his or her life, or cause disfigurement, physical impairment, or undue discomfort if delayed. This authority is granted after a reasonable effort has been made to reach me.

Photo/Video Consent
I consent for my child to appear in photos and videos taken during Adventure Clubs that may be used on lakesidebiblechurch.com, in printed promotional materials, or on the social media pages operated by Lakeside Bible Church. I waive all claims to compensation for any of the aforementioned media.
